I hereby acknowledge the following terms and conditions set forth by PARTY PROS, Inc., for the rental and use of the tent, tables, chairs, linens, or inflatable equipment:

1. Assumption of Risk: I understand and acknowledge that the use of inflatable equipment involves inherent risks, including but not limited to, falling, slipping, colliding with other participants, and other potential injuries. I voluntarily assume all risks associated with the use of the inflatable equipment.

2. Safety Guidelines: I agree to follow all safety guidelines provided by PARTY PROS, Inc. and its employees. I understand that failure to comply with these guidelines may result in injury to myself or others. I will ensure that all participants using the tables, chairs, or inflatable equipment are aware of and adhere to these safety guidelines,

3. Supervision: I understand that it is my responsibility to supervise all participants using the inflatable equipment at all times. I will ensure that there is a responsible adult present to supervise the activity and enforce safety rules at all times while inflated.

4. Release of Liability: In consideration of being allowed to rent and use the tables, chairs, or inflatable equipment, I hereby release, discharge, and hold harmless PARTY PROS, Inc. its employees, agents and assigns from any and all claims. demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any participant while using the tables. chairs, or inflatable equipment.

5. Indemnification: I agree to indemnify and hold harmless PARTY PROS, Inc., its employees, agents and assigns from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by any third party as a result of my use of the tables, chairs, or inflatable equipment,

6. Damages: Any damages to our tents, tables, chairs, linens, or inflatables will be subject to a fee based off of the value of the item and the damage.

7. Overnight Rentals: Renter understands and acknowledges that the blowers are to be removed from inflatable device and locked up in a secure area.

8. Agreement to Terms: I have carefully read and fully understand the terms and conditions of this waiver, acknowledge that I am voluntarily assuming all risks associated with the use of the tables, chairs, or inflatable equipment and agree to abide by all safety guidelines provided by PARTY PROS, Inc. including the Safety Rules in the attached Safety Checklist.

WAIVER OF LIABILITY

•  I have been shown how inflatable is secured and shown how the blower is turned on and off and how it is removed from the inflatable. 

•  In the event of high winds and storms, I have been instructed to get all participants off the inflatable and unplug the extension cord from the power outlet. 

•  I have been instructed not to allow horseplay of any kind, flipping, wrestling or any other unsafe activities both inside and around the inflatable. 

•  I understand that an adult (18 year old +) operators must be present to supervise the inflatable activities at all times. 

•  I have been advised that children of the same size or age group only may use the inflatable together. No adults and children at the same time. 

•  Maximum number of participants of each group that should play in the unit together, is 6 participants with a maximum of 600 lbs. Height requirements: 36” minimum; maximum 72” height. 

•  Inflatables MUST be securely anchored and fully inflated BEFORE any participants enter. 

•  I agree to remove any participant who is violating these POSTED rules of operation. 

•  I have received both written and verbal instructions on the safe operation of the inflatable. 

•  Under no circumstances is climbing on the outside of the inflatable permitted.
